SIGN · PDF COPY · REGISTER · RENEW
Survtapp collects signed liability waivers on iPads, Android tablets and the web. Participants sign on-screen and receive a PDF copy by email, and every signed waiver lands in a searchable register with its timestamps and the exact text that was presented. The free plan includes 100 signed waivers a month.

Build the waiver once — headings, paragraphs, checkboxes, initials and a signature block — or start from a template. Participants sign on a tablet at your front desk, on a kiosk-locked device, or from a link on their own phone. The signer gets a PDF copy by email; the signed record goes to your register.
Kiosk mode locks a tablet to the waiver so customers cannot wander into other apps, and signing keeps working offline: signatures captured in a dead zone queue on the device and sync when the connection returns.

The signature and initials, the signing timestamps, the IP address, every consent checkbox as it was answered — and the exact waiver text as it was presented to the signer, retained with the record. If you edit your waiver next season, previously signed records keep the wording their signers actually saw.
Records are cryptographically integrity-checked, so any later alteration is detectable, and each signed waiver renders to a PDF that shows what was agreed to.

The Waiver Register lists every signed waiver in one searchable place. Waivers carry a validity period you set — a season, a year, a single visit — and Survtapp tracks expiry, sends renewal reminders, and lets you re-request a signature when a waiver lapses or the wording changes.
Signed waivers are private by default. A signed legal record never sits on the open web unless you deliberately share it.

Learn what a release of liability covers, start from the free liability waiver template or the waiver generator, collect signatures unattended with kiosk mode, or collect consent forms the same way. Plans are on the pricing page.
Yes — share the waiver as a link or QR code and participants sign from their own phone. Walk-ins sign on your tablet or kiosk when they arrive.
Yes. Signatures captured while a device is offline are stored locally with their timestamps and sync automatically when the connection returns.
Yes — a PDF copy of the signed waiver is emailed to the signer automatically.
Signed records keep the exact text that was presented when they were signed. New signers see — and are bound to — the new wording; old records are unaffected.
Each waiver has a validity period you choose. Survtapp tracks expiry in the register, sends renewal reminders, and can request a fresh signature when a waiver lapses.
Electronic signatures are widely recognized in Canada, the US and many other jurisdictions, but enforceability depends on your jurisdiction and how the waiver is drafted. Survtapp records the signature, timestamps, IP and the exact text presented so your records are complete. This is general information, not legal advice.
