What counts as a consent form?
Photo and media releases, activity and participation consent, treatment and procedure consent, field-trip permissions, research participation — any document where a person agrees to something specific in writing. In Survtapp they are built like waivers: your text, explicit checkboxes for each consent, initials where you want emphasis, and a signature.
Why does the exact wording matter for consent?
Consent is only as good as the record of what was agreed to. Survtapp stores the consent text and version against every signed record — so when your form changes, each record still shows the wording its signer actually saw, with the signing timestamps and IP alongside. Records are integrity-checked, making later alteration detectable.
That version history matters in regulated settings: when a policy update changes your consent language, you can see exactly who agreed to which version, and re-request consent from the people on the old one.
How do people sign?
On a tablet at reception, on a kiosk-locked device, or from a link or QR code on their own phone — online or offline, with offline signatures syncing automatically. The signer receives a PDF copy by email; your team sees every signed form in a searchable register with expiry and renewal tracking.

Yes — share the form as a link or QR code. On site, a tablet or kiosk-locked device works for walk-ins.
Does it work offline?
Yes. Consent captured offline queues on the device with its timestamps and syncs automatically when the connection returns.
Does the person get a copy of what they signed?
Yes — a PDF copy is emailed to the signer automatically.
How is consent kept private?
Signed forms are private by default and hosted in Canada. They are never on the open web unless you deliberately share one, and your team accesses them through your account’s roles and permissions.
